Daily Health Screening

Every day, NYC Schools require that all students have a printed or electronic copy of a completed health screening questionnaire.

If your student has a smartphone, you can set a shortcut or bookmark to the NYCDOE health screening page: https://healthscreening.schools.nyc. To log in, use your student's DOE Student Account username and password (managed here: https://idp.nycenet.edu). You or your student can complete the form before school: just show the completed form on the phone.

Alternatively, you can download this PDF and print out the questionnaire at home to be completed and brought to school each day.
