NYC School Survey (due Friday April 15th)

Don’t miss the opportunity to provide your school with valuable feedback! Please be honest, it’s important for our school to know areas they need to improve and know areas they are doing great in.

Complete the NYC School Survey online at NYCSchoolSurvey.org or by logging into your NYC Schools Account today.

The deadline to complete the survey has been extended to Friday, April 15!

If your child is in grades K–12, you will need your child’s nine-digit student identification number, or OSIS, to begin filling out your survey online. You can find the OSIS on your student’s report card, school ID card, or on your NYC Schools Account. On the survey page, you must enter the letter f, followed by your child’s OSIS to access the survey. Note that the “f” must be lowercase (e.g. f123456789).

If your child is in 3-K or Pre-K, please contact your child’s school or program to receive your survey access code.

You can also take the survey by logging on to your NYC Schools Account.
